接触模式改变中国COVID-19爆发的动态

复旦大学余宏杰、意大利Bruno Kessler基金会Marco Ajelli等研究人员合作发现,接触模式的改变影响了中国COVID-19爆发的动态。这一研究成果于2020年4月29日在线发表在《科学》上。

研究人员表示,中国采取了强有力的非药物干预措施,以阻止新型冠状病毒疾病(COVID-19)的传播。随着其他国家传播的加剧,年龄、接触方式、社交距离、感染易感性和COVID-19动态之间的相互作用尚不清楚。
 
为了回答这些问题,研究人员分析了爆发前后武汉和上海的接触者调查数据以及湖南省的接触者追踪信息。在COVID-19远离社交的期间,每天的联系减少了7-8倍,大部分互动仅限于家庭。
 
研究人员发现,0-14岁的儿童比15-64岁的成年人更不容易感染SARS-CoV-2(比值0.34,95%CI 0.24-0.49),而65岁以上的人更容易感染SARS-CoV-2感染率(比值比1.47,95%CI:1.12-1.92)。
 
基于这些数据,研究人员建立了一个传播模型来研究社交距离和学校关闭对传播的影响。他们发现,仅在暴发期间在中国实施的社交距离就足以控制COVID-19。主动关闭学校并不能自行中断传播,但可以将高峰发病率降低40-60%,并延迟流行。

Title: Changes in contact patterns shape the dynamics of the COVID-19 outbreak in China

Abstract: Abstract Intense non-pharmaceutical interventions were put in place in China to stop transmission of the novel coronavirus disease (COVID-19). As transmission intensifies in other countries, the interplay between age, contact patterns, social distancing, susceptibility to infection, and COVID-19 dynamics remains unclear. To answer these questions, we analyze contact surveys data for Wuhan and Shanghai before and during the outbreak and contact tracing information from Hunan Province. Daily contacts were reduced 7-8-fold during the COVID-19 social distancing period, with most interactions restricted to the household. We find that children 0-14 years are less susceptible to SARS-CoV-2 infection than adults 15-64 years of age (odd ratio 0.34, 95%CI 0.24-0.49), while in contrast, individuals over 65 years are more susceptible to infection (odd ratio 1.47, 95%CI: 1.12-1.92). Based on these data, we build a transmission model to study the impact of social distancing and school closure on transmission. We find that social distancing alone, as implemented in China during the outbreak, is sufficient to control COVID-19. While proactive school closures cannot interrupt transmission on their own, they can reduce peak incidence by 40-60% and delay the epidemic.
